The Provided Fields page contains some of the most common fields that would usually be on a form.
Click the checkbox beside each field you want to include on your form. You can
make the field "Required" on your form by clicking on the "Required" checkbox beside each field.
After you have chosen the Provided Fields you want, click the submit button to add the
fields to your form. You can add and remove the Provided Fields on this page as needed.
You can view your form by clicking on the "View Form" link. You can update the View
Form page by clicking on the "View Form" link or by clicking on the "Refresh Form" link below
the form on the View Form page.
If you need to create your own custom fields you will be able to add those on the next
page of the Wizard once you have completed choosing the Provided Fields.
All the fields will appear on your form in the order they are listed on the Provided Fields page
and any of your own custom fields that you create later will follow the Provided Fields that you choose.
When you are finished choosing your fields click the "Continue" button to proceed.
